Every patient is responsible for providing, to the best of his or her ability, complete information about their current health and past medical history. It is very important for us to know about your behavioral concerns, general health issues, medications, illnesses, and hospitalizations. Additionally, any recent unexpected changes in condition and other matters relating to your health should be indicated. We ask that you download the two patient forms and submit these, either electronically by email or regular mail, four weeks prior to your initial office visit.

In order to ensure an objective and scientific medical diagnosis, we ask that you fulfill a complete lab analysis prior to your office visit. Our subjective and experienced-based observation methods are strengthened when the patient asks questions when something is not understood about care or treatment. You will need to know what actions are expected of you and participate in your path toward wellness. It is the patient's responsibility to follow the care or treatment plan developed. If you do not, you are accepting the responsibility for the consequences. Additionally, our clinic cancellation policy requires that you provide a 24-hour notice in the event that you need to cancel or reschedule any office appointment.
